Who uses this template
Practice managers, clinic founders, and healthcare operators use this to open a medical or dental clinic on time. It maps the licensing pathway, clinician credentialing, and payer enrolment alongside medical-gas and infection-control fit-out and inspections, so the long regulatory lead items progress in parallel with the build toward first appointments.
How to customize it
- Start the credentialing bar at the offer date and give it your payers' real turnaround, not the best case. If it finishes after the licence, credentialing is your critical path and the chart should say so.
- Add one row per clinician if you are opening with several. They do not all clear at the same time, and the opening date depends on the slowest one, not the average.
- Separate payer enrolment from credentialing. Being privileged to practise and being contracted to bill are different processes with different clocks, and the second one determines revenue, not access.
- Keep medical gas piping, verification and certification as their own bars. It is a specialist installation with a third-party verifier and it cannot be absorbed into general mechanical works.
- Add the infection control risk assessment as a real activity with a close-out, especially if you are fitting out inside an occupied building.
- Put the mock patient day in before go-live and staff it fully. It is the only rehearsal you get of check-in, rooming, the EHR, the equipment and the claim, all in one flow.
Scheduling tips
- Confirm the licensing pathway before you sign the lease. Whether you need a certificate of need, which licence class applies and what plan review requires can change the space, the build cost and the timeline entirely.
- Submit credentialing files complete or not at all. An incomplete file does not start the clock, it goes to the back of the queue when it is corrected, and that single administrative slip is the most common cause of a delayed opening.
- Design for plan review, not just for the builder. Health authority plan review has specific requirements on room sizes, sinks, pressure relationships and finishes, and a redesign after a review comment costs far more than getting it right first.
- Book the fire marshal and health department inspections early and expect a re-inspection. Their diaries are not yours, and scheduling a go-live that assumes a first-time pass on both is a bet you will usually lose.
- Commission and calibrate equipment with the biomed team, not the vendor alone. Delivery and installation are not the same as being calibrated, surveyed and recorded on an asset register the inspector will ask to see.
- Run test claims through the clearinghouse before the first real patient. Discovering a broken payer configuration in week one means seeing patients you cannot bill for, which is worse than opening a week later.

How long does it take to open a medical clinic?
Twelve to sixteen months is typical for a fitted-out clinic in leased space, which is roughly what the template uses. The build is rarely the constraint; licensing and credentialing are.
How long does clinician credentialing take?
Commonly 90-180 days from a complete file, and longer if payer enrolment is included. It is run by verification organisations, medical staff committees, state boards and payers, so it should be started at the offer date and tracked as a long-lead item.
What makes a medical fit-out different from a commercial one?
Medical gas piping with third-party verification, HVAC pressure relationships between clean and soiled areas, infection-control barriers during construction, cleanable finishes and shielding for imaging rooms, none of which appear in ordinary office fit-out.
Can we open before payer contracts are effective?
Physically yes, financially no. Seeing patients you cannot bill for is a common and expensive mistake, so payer contracts effective is treated as its own milestone rather than being folded into credentialing.
What inspections are needed before opening?
Typically fire marshal and building occupancy, a health department clinic licensing inspection, medical gas verification, a radiation physicist survey where imaging is installed, and CLIA certification for any in-house laboratory.
